  • Students who wish to enroll in the Student Health Insurance Plan must submit an online enrollment form. All eligible students are charged for the plan upon course registration.

    The student health insurance enrollment form for the 2021-2022 school year will be available at universityhealthplans.com beginning May 5, 2021. The deadline to submit an enrollment or waiver form is September 13, 2021.

    2021-2022 School Year

    • The fall semester coverage period is August 20, 2021-January 14, 2022. The spring semester coverage period is January 15, 2022-August 19, 2022.
    • You can complete an enrollment form at any time before the enrollment/waiver deadline; however, you will not be officially enrolled in the plan until you are registered for classes.
    • We recommend that you submit the enrollment form at least two weeks before August 20. This will ensure that you receive proof of insurance by the plan’s August 20 start date and can start using your insurance right away, as long as you are registered for classes.
    • When you elect to enroll in the Student Health Insurance Plan in the fall, you will be automatically enrolled in the spring semester, provided you are a registered student in the spring. If you are not registered in the spring, your insurance coverage will automatically terminate at the end of the fall semester coverage period (January 14).
    • If you graduate in the fall semester, your insurance coverage will terminate on January 14. You (and any enrolled dependents) will not be eligible for insurance coverage for the spring semester, since you will not be a registered student in the spring.

    2020-2021 School Year

    • The open enrollment period for the 2020-2021 school year has passed.
    • Students experiencing a qualifying life event, such as loss of private insurance coverage, can request to enroll in the Student Health Insurance Plan mid-semester for a prorated premium by completing a Petition to Add form on Gallagher’s website.
